If you are looking to register for an Anger Management Class, a Family Violence Class, a Substance Abuse Evaluation, or a Clinical Evaluation, you would have to call our office at (706) 549-7755 in order to get the correct forms sent to you. We do not perform Mental Health Evaluations, and we are not traditional Medicaid or Medicare providers. If you have any other questions or concerns you can always give our office a call and we'd be more than happy to help you!
Affordable Fees
A unique aspect of our mission is to offer a reduced or “sliding” fee scale for clients without insurance benefits. These reduced fees are made possible through donations from grants and the generosity of individual donors. Your fee is determined based on your gross annual household income. We also accept cash, check, VISA, MasterCard, and American Express and our staff are credentialed providers for most major insurance plans. If you are using insurance we can let you know which providers are covered by your plan and we will file the claims for you. Please note that there is no guarantee of coverage or claim payment when using insurance. You will be responsible for any co-pays or deductibles at the time of service. Please call our business office Monday-Thursday 9am-5pm and Friday 9am-4pm to speak with our Intake Coordinator who will explain our affordable fee options based on your needs. Appointments may be scheduled by calling 706-549-7755.
